No, I wasn't using InfiniteAmmo, also I'm pretty sure I didn't drop/threw it, also it was on the end of the round, so I had time for looking around searching the item but didn't find it.

How about placing more than one accesory on a character?

not edit at all, but you can move tiles in the editor in official map if you move it and without releasing left mouse button you press the right mouse button(the menu opens), and then click anywhere on the screen, then you can actually move the tile. On the upper panel, you'll find many icons, there's a magnet, a grid, a cross, a green square, and the last one is a path node. You have to click on this last icon in order to hide the outlines.
