While what you said about the weapon pool is correct, you should also mention that it is pretty polluted with redundant weapons that function too similarly to existing ones. You can use the A+D grab method in a way to first block an attack and then initiate a grab quick enough to grab your opponent before they can even land their second punch. I don't see why this isn't activated for such situations, it's fitting and it means the team with more players will go on the offensive which they probably have the option to do.

Similar topics have already been brought up some times in the past and our answer is the same: SFD isn't deterministic in events or simulation in physics. We're are aware that the box2D engine likes to... get janky... a lot when resolving overlap sometimes. Rolling is programmed to have a small chance to fail, I believe, so this wouldn't be a problem/bug
That only applies to dodging bullets, not whether you put out fire or not.
